Travel Release Form

It is the procedure of the Athletic Department of Butler High School that athletes travel to and from away athletic contests with their team in school provided transportation.  We understand that extenuating circumstances do occur where it may be necessary for an athlete to return home from an athletic contest with a parent rather than with his/her team.  Such occurences should be for matters of an emergent nature and should be a one time occurence.  An athlete may not drive him/herself from an event and may only be picked up by his/her parent.

The Travel Release form below needs to be completed, signed and returned to the athlete's coach one day before the event from which the athlete is not traveling back to BHS with his/her team.

Physical Examination

Students registering for Athletics must have a current physical examination and a completed Health History questionnaire on file in the nurse's office before the student is authorized to participate in practice/tryouts. Physicals are valid for *12 months* from the date of the last exam. Print and complete the Health History Update if the Student`s last physical was 90 days prior to the first day of practice. Print the Physical and Medical Clearance Forms and once completed by the parent and physician, return to the athletic department.
